Özet

Nükleoplasti; diskojenik ağrıya neden olan geniş tabanlı disk çıkıntısının tedavisinde intradiskal basıncı azaltmak için kullanılan intradiskal ısıtma yöntemidir. İşlem sonrası hastalarda sinir kökü yaralanması, dural hasar, enfeksiyon gibi komplikasyonlar bildirilmiştir. Her iki olgu da nükleoplasti işleminden haftalar sonra şiddetli bel ağrısı ile başvurdu. Hastaların nörolojik defisiti olmamasına rağmen fiziksel hareketlerinde ciddi kısıtlılık vardı. En ufak bir sarsıntıda daha yoğun hale gelen bel ağrısı görüldü. Radyolojik olarak, MR görüntülerinde intervertebral disk alanında enfeksiyonla ilişkili bir değişiklik tespit edildi. Başlangıçta abse drenajı bir hastada uzak lateral yaklaşımla, ikincisinde ise paramedian yaklaşımla yeterliydi. Paramedian abse drenajı uygulanan hastada iyatrojenik instabilite nedeniyle takip eden ayın sonunda posterior vertebral stabilizasyon gerekti. Minimal invaziv olduğu düşünülen nükleoplasti gibi perkütan bir tekniğin spondilodiskit şeklinde ciddi bir enfeksiyona neden olabileceğini 2 olgu bildirisi ile vurgulamak istedik.

Abstract

Nucleoplasty; is an intradiscal warming method used to reduce intradiscal pressure in the treatment of wide-based disc protrusion causing discogenic pain. Complications such as nerve root injury, dural damage, and infection have been reported in patients after the procedure. Both cases presented with severe low back pain weeks after the nucleoplasty procedure. Although the patients did not have neurological deficits, there was a severe limitation in their physical movements. Low back pain, which became more intense at the slightest jolt, was observed. Radiologically, an infection-related change in intervertebral disc space was detected with MR images. Initially, abscess drainage was sufficient in one patient with a far lateral approach alone and in the second with a paramedian approach. Posterior vertebral stabilization was required at the end of the following month due to iatrogenic instability in the patient who underwent paramedian abscess drainage. We wanted to emphasize with 2 case reports that a percutaneous technique such as nucleoplasty, which is considered to be minimally invasive, can cause a serious infection in the form of spondylodiscitis.
